Feel free to leave any blanks where you have questions or contact me if something is unclear. A copy of the Privacy Practices is available upon request or can be printed from the website.

Required For All Clients

• Initial Paperwork - Please complete relevant information.

Complete ONLY If Using the Magellan EAP

• Magellan Statement of Understanding - This document is required by Magellan. Only the person who called Magellan to set up the EAP authorization should complete this.

Complete ONLY If Using the Chestnut EAP

• Case Manager Intake Form - This document is required to be completed and returned to Chestnut. Complete the top portion of the document under the "Client Information" heading.

Complete ONLY If Using the Personal Assistance Services (PAS) EAP

• PAS Consent to Treat - This document is required by PAS. Complete the blanks and sign the form.

Complete ONLY If Using the Resources Management Services (RMS) EAP

• RMS Statement of Understanding - This document is required by RMS. Complete the blanks and sign the form.
